Event Reports

Overview

As part of our Canadian Nuclear Safety Commission regulations, TRIUMF routinely updates and publishes a Public Disclosure Protocol (PIDP), which addresses public interests in relation to licensed activities.

As part of our commitment to regulatory excellence, and our TRIUMF values of safety and transparency, TRIUMF publishes a list of Event Reports that describe any events that relate to our operations and activities that may be of interest to the public.

Event reports - 2025

  • February 21, 2025: In response to an earthquake that occurred at approximately 1:26 PM, and out of an abundance of caution, TRIUMF briefly activated and then de-activated its Emergency Response Plan. After review, there were no impacts to the TRIUMF community; facility infrastructure; safety or environmental monitoring systems; or the environment.

 

  • July 25, 2025: A low-impact, unexpected radiation exposure event occurred during the routine production of medical isotopes at TRIUMF, when a production target fell outside its shielded flask. Relevant safety protocols were initiated, and as an extra precaution, the main cyclotron was briefly de-activated following the incident. Ultimately, the situation was resolved without any impact to TRIUMF staff or the wider local community, and at no time did exposure exceed regulatory limits. TRIUMF continues to investigate the root cause of the incident, and corrective actions are forthcoming.
